El presidente Polk nombra a Elisha Bailey «cirujano adjunto al servicio de los Estados Unidos». Firmado con pulcritud al final con tinta por el presidente James K. Polk y refrendado por el secretario de Guerra, William L. Marcy. El sello de papel blanco colocado en la esquina superior izquierda permanece intacto. En muy buen estado o excelente, con pliegues entrecruzados y marcas de tinta que afectan a partes de la parte superior e inferior.
