Präsident Coolidge ernennt Mary Tempchin „zur Notarin für den District of Columbia“. Am Ende mit Füllfederhalter von Präsident Calvin Coolidge unterzeichnet und von Generalstaatsanwalt John G. Sargent gegengezeichnet. Das unten links angebrachte Prägesiegel ist intakt. In sehr gutem Zustand, mit vertikalen und horizontalen Falten in der Mitte.
